* Are a Cooperative formed under Wisconsin Co-op care law, owned by its member organizations, operating under its bylaws, overseen by a nine-member board of directors, relying on volunteer committees and assisted by a small staff.

* Offer fully insured, high-deductibel health reimbursement arrangement or health savings account qualified plans, believing that having control over your healthcare dollars will in turn motivate good decisions on sick-care use. The HLC offers a constant flow of information to improve understanding and utilization of sick care and insurance systems to help you make efficient, effective decisions.

* Motivate people to lead healthier lifestyles, believing that the key to stabilizing insurance costs is to decrease high-cost preventable illness and disease. Each of us does this by getting healthier year to year. Essential to this is knowing our health status via yearly health risk assessments, offered through the HLC, and doing what is necessary to continuously improve health status.

* Require employers participating in the insurance program to commit to three years. An insurer will not work with an organization if it knows a substantial part of the insured base can leave at whim.

Membership requirements:

* Open to any self-employed person, any for-profit or non-profit corporation, organization, any municipality or any partnerships.
* Must do business in, be located in, have a principal office in or reside in Brown County, Wisconsin
* Must be a member of a local farm cooperative, Green Bay Area Chamber of Commerce or the Non Profit Resource Group (NPRG)
* Must complete an HLC membership application form and submit along wtih a non-refundable membership fee
